Bloomingdale residents may register for the annual SMAC Test starting on Monday, October 1, 2007 at the Municipal Building, 101 Hamburg Turnpike at the Borough Clerk’s Office

Appointments may be made when the form is completed. A SASE (self addressed stamped envelope) and the full name, address, and telephone number of your physician is required to participate in this screening.

Cash or a check for $20 should be made payable to the “Borough of Bloomingdale”. A receipt will be given at the time the appointment is made.

The test will be held on Thursday, November 1, from 7AM to 10 AM at the Fireman’s hall next to the Municipal Building . A 12 hour fast is necessary. Early appointments are reserved for residents with health concerns and those whop are actively employed.
